Whether we are preparing you for an upcoming surgery or helping you recover from a serious injury or stroke, you can trust that the team at Beyond Limits is always here for you. By developing a treatment plan focused on your individual goals, we can help you overcome whatever your limitations so that you once again live the healthy, active lifestyle you love. As you begin your journey on the road to recovery, here are few things you might need to know:

Q: Are rehabilitation services covered by insurance?

Most insurance policies cover rehabilitation services when provided by a licensed therapy professional or therapy assistant. We will verify your insurance benefits as a courtesy to you. We rely on your insurance company to give us proper information, but we cannot guarantee accuracy. If you feel there is a discrepancy in your benefits or coverage, please bring it to our attention or contact your insurance company. If your insurance requires pre-authorization and/or a referral for physical therapy, it will be the patient’s responsibility to ensure that the referring physician or primary care physician has obtained the necessary pre-authorization.

We require that copayment, deductibles, and supply charges are collected at the time of service and that the patient remits payment for any balance due to continue services. If you have any questions regarding this process, please speak to our billing specialist.

Q: What should I wear?

For physical therapy or occupational therapy, you should wear comfortable clothes that are not restrictive to movement and closed-toe shoes like sneakers. You may want to wear something that allows us to access the body part requiring treatment without having to change. For ease of treatment with a shoulder injury, please wear a shirt that buttons in front or allows access to the treatment area. If needed, we can provide you with a patient gown. For speech therapy, generally no special apparel is necessary.

Q: How long will my evaluation take?

Evaluations usually take one hour, but may take up to two hours if treatment is initiated.

Q: What forms will NEW patients need to fill out and/or provide?

As a new patient, you will need to fill out forms for: Medical History, Insurance & Patient Responsibility, Notice of Privacy Practices, and Consent for Services. As part of your intake process, you may also be asked to complete additional forms upon your arrival so that we can further understand you needs.

Q: What forms will RETURNING patients need to fill out and/or provide?

Depending on how long it’s been since you were in to see us, we may need you to update some forms. If you’d like to complete them ahead of your appointment, please give us a call and we can let you know what will be required.
